A FSGC clearance must be obtained for any adult who plans to volunteer for activities involving the youth of our parish, school, and/or CYM sports.
You must complete all 3 steps for your FSGC Clearance:
A FSGC Clearance must be obtained for any adult who plans to volunteer for activities involving the youth of our parish, school, and/or CYM sports.
Instructions are listed below and all three steps must be completed for you to be cleared. FYI, clearance processing time is 3-5 business days.
Step 1: Submit a Background Clearance. To apply for or renew a background clearance, email the FSGC Compliance Coordinator Kylie Lavelle at [email protected] with your name and request for a prepaid access code. When emailing, briefly describe the nature of your volunteer request (school, CYM sports, etc). The code will allow you to access InfoRetrieval Services directly on their website and apply online. The access code will be specific for each individual and be good for one application. Your access code and link to the InfoRetrieval Services Website will be emailed to you.
Step 2: Watch Videos. You must view the educational videos by visiting the Diocesan website: https://cdow.org/ministries/safe-environment/for-the-sake-of-gods-children/. After viewing all the videos, please be sure to click on “register with the FSGC office”. Please forward the confirmation email directly to [email protected].
Step 3: Fill out Forms. A Volunteer Covenant Form & Acceptable Use of Technology Form needs to be filled out & on file in the parish or school office. (Forms can be scanned and sent to the parish office attention: Kylie Lavelle).

If you have completed a background check with another parish or school within our diocese, those clearances can be transferred over to SMM. Please let us know.

In the summer of 2002 three Task Forces at the Diocesan level were appointed to develop recommended policies, procedures and protocols to create and maintain safe environments. The Task Forces have completed their work and their recommendations have been accepted by the Bishop. Our responsibility is to share with you what this will mean to our parish community, in our programs and all of our activities with children and young people.
Article 12 of the Charter states: Dioceses will establish “safe environment” programs. They will cooperate with parents, civil authorities, educators and community organizations to provide education and training for children, youth, parents, ministers, educators and others about ways to make and maintain a safe environment for children. The Diocese will make clear to clergy and all members of the community the standards of conduct for clergy and other persons in positions of trust with regard to sexual abuse. The Diocesan program entitled “For the Sake of God’s Children” presents a three pronged response to the objectives set forth in articles of the Charter: Background Checks for adults who have regular contact with the young people of our community, Ethical Standards for all Church Personnel whether compensated or volunteers and strategies for creating/maintaining Safe Environments in our activities and facilities.
